Use this FAFSA worksheet prior to completing your FAFSA on the web.  This form follows the FAFSA on the web, which will allow you to transfer information to the online application, available after January 1, 2008.  DO NOT MAIL THIS WORKSHEET TO FAFSA.

^{2) FERPA - Release of Information to Designated 3rd Party}

 

Use this form to authorize release of Financial Aid information to a specific, designated 3rd party.  We are unable to release information to anyone without the written permission of the student.

^{3) Financial Aid Appeal Request Form}
 

Use this form to request reinstatement of financial aid after you have been suspended from Financial Aid for not making Satisfactory Academic Progress.  You must also attach a letter, and supporting documentation to have your request considered.

^{5) Income/Expense Verification Form}

The following form may be requested during the verification process to verify income and expenses for students that report little or no income.  Please do not submit this form to the Financial Aid Office unless requested by a Financial Aid Counselor.

^{6) Loan Exit Interview}

Please click the link below to view instructions and complete online Loan Exit Counseling.  Loan Exit Counseling is required of ALL federal loan borrowers who are filing a Petition to Graduate, leaving HPU, or dropping below half-time attendance (summer session excluded).

^{8) Military Benefits Form}

The Military Benefits form must be completed by every student who receives military benefits (BAH, BAS, COLA), or whose spouse or (in the case of Dependent students) parents receive military benefits. Please complete this form and return it to the financial aid office in order to be reviewed for financial aid.

This form is used by students who are ineligible to file, or are waiving their right to file the Free Application for Federal Student Aid (FAFSA), but will be working with the Financial Aid Office on other financing, including, but not limited to alternative loans.

^{14) PLUS Loan Authorization Form}

By signing and returning this form, Parents are authorizing HPU to release excess funds from your parent PLUS loan to your son or daughter.  This form must be renewed each academic year.

Some financial aid applications are selected for review in a process called "Verification". In this process, our financial aid office will compare information from your application with signed copies of your and your parent(s) 2008 Federal Tax Forms, W2s and the 2009-2010 Verification Worksheet for the 2009-2010 school year. Please complete the attached Verification Worksheet and send them in to our office with your taxes and W-2s.
