Medical Requirements and Health Care Information

MEDICAL REQUIREMENTS

Measles Immunizations
The state of Hawaii requires all students to be immunized for measles (rubeola). Two doses of live vaccine or two doses of MMR separated by at least one month are required. First dose must have been given after January 1, 1968.

Measles immunization may be waived if:

  • Student is born before 1957 (provide proof of birth date)
  • Physician had confirmed diagnosis in the past or serologic evidence of immunity is presented (provide written statement from your physician; statement must be signed and written on official stationery).

NOTE: First-time international students may have their measles (MMR) immunizations taken in their home country.

TB Clearance
Hawaii Department of Health regulations require that students in all schools provide a tuberculosis clearance issued in the United States. Clearance must have been given within 12 months prior to first enrollment in a postsecondary institution. Continuing spring 2008 students MUST have a TB clearance PRIOR to receiving a registration clearance.

International students and U.S. citizens living overseas must complete the Tuberculin Skin Test immediately upon entry into the U.S. TB certification issued outside of the U.S. cannot be accepted by the University.

A TB clearance may be obtained from any of the following:

  • A U.S. physician; refer to your health insurance plan.
  • KPHC Downtown (Family Medical & Dental Center): Located on 89 South King Street, Honolulu (near the HPU downtown campus). Open Monday through Friday from 8am to 5pm. Call (808) 792-5570. Fee: Contact KPHC for cost.
  • Lanakila Health Clinic: Department of Health TB Testing Clinic, 1700 Lanakila Avenue. Testing is every weekday except Thursday, 7:45am - 3:00pm (closed 12pm-1pm). For chest x-ray, Monday through Friday, 7:45am - 3:00pm, and on Thursdays, 7:45am-2pm. Call (808) 832-5731. Fee: No Charge.
  • Diamond Head Health Center, 3627 Kilauea Avenue, Room 302, Honolulu, HI 96816. Tuesdays, 2pm - 4pm. Call (808) 733-9220. This location is convenient for those living in east Honolulu. Fee: No Charge.
  • Windward Health Center, 45-691 Keaahala Road, Kaneohe, HI 96744. Tuesdays, 1:30pm - 3:30pm. Call (808) 233-5450. This location is convenient for those living on the Windard side of Oahu. Fee: No Charge.

Attention International Students

All full-time international students are REQUIRED to either purchase one of the HPU plans or obtain a waiver. 

 If you wish to obtain a waiver, your medical insurance must meet the following requirements: 

  • Valid coverage dates for the entire term
  • Coverage for outpatient care, hospitalization, emergency room, accidents, medical and surgery in Hawaii
  • Non-reimbursement policy
  • Minimum $100,000 USD coverage
  • Medical benefits of at least $50,000 per accident or illness
  • A deductible not to exceed $500 per accident or illness
  • At least 75% coverage for each accident or illness

Additional requirements for International Students:

  • Repatriation of remains in the amount of $7,500
  • Expenses associated with the medical evacuation of the exchange visitor to his or her home country in the amount of $10,000

Plans that are NOT accepted include:

  • Life insurance policies
  • Travel insurance plans
  • Accident or hospitalization only plans
  • Reimbursement policies

To obtain a waiver, international students must submit proof of medical coverage (meeting the above stated requirements) in ENGLISH only. Please take care of this BEFORE the semester begins.

Attention All Nursing Students
The College of Nursing and Health Sciences require health insurance coverage for all nursing students in nursing course levels one through five and all nursing courses for MSN students.  Each student will be required to provide proof of insurance coverage by health requirement deadline. Contact the college for details at (808) 236-3552.

Attention Hawai'i Loa Campus Residents
Residents are required to provide proof of personal medical health insurance throughout the duration of their housing contract. A resident’s health insurance must be valid in Hawai‘i. For details, please contact the staff directly at (808)236-3540.

MEDICAL INSURANCE PLANS AT HPU  

We strongly urge all HPU students to have medical insurance.  If you are a student from the U.S. mainland, contact your insurance carrier regarding the scope of coverage in Hawaii.  

Department of Homeland Security regulations require that colleges and universities certify that international students have sufficient funds on hand to defray expenses in the event of an emergency. HPU has determined that a mandatory health care program best satisfies this requirement for our international students. Hence, all full-time international students are REQUIRED to either purchase one of the HPU plans or obtain a waiver.

We are pleased to offer HMSA (Blue Cross-Blue Shield Association) and Kaiser medical insurance plans.  

Who is eligible?

  • HPU students enrolled at least half-time.
  •  Students participating in approved HPU sponsored programs.
  •  HPU visiting colleagues, short-term scholars/professors.
  • HPU students doing Optional Practical Training (OPT).

Hawaii Medical Services Association (HMSA) - Blue Cross-Blue Shield Association
With the HMSA Plan, students have the freedom to choose the provider who best meets their health care needs. HMSA's plan includes drug and vision benefits for HPU participants.

You can receive care from any HMSA participating provider statewide. HMSA's network includes 95% of all health care providers in the state and all major hospitals. There are many HMSA health care providers near the HPU downtown and Hawaii Loa campuses such as Queen's Medical Center, Straub Clinic and Hospital, and Kalihi-Palama Health Center (downtown Honolulu). Kaiser Foundation
The Kaiser Plan is an HMO plan that makes it easy for HPU students to get their medical services at convenient Kaiser locations statewide--11 locations on Oahu, five on Maui, and three on the Big Island of Hawaii. Students can choose or change their Primary Care Physician (PCP) at any time and are welcome to make same-day appointments at any Kaiser Clinic. Kaiser's coverage includes drug and vision benefits.

The Honolulu Clinic (1010 Pensacola Street) is conveniently located within 15 minutes of the HPU downtown campus. Access to the parking structure, as well as the building, is available on both King and Pensacola Streets. In addition, the clinic is situated directly along city bus routes both to and from the downtown campus. Bus directions from the HPU downtown campus: on Hotel Street (east-bound), take Bus #1 (all routes), #2, #13 to the Honolulu Clinic. Dental Plan
Offered to HPU Students

 HPU students now have the option of purchasing a student dental plan. This student plan is a dental discount program provided directly to you by Hawaii Family Dental Center dentists. The dental plan is flexible and may be purchased for a single or multiple semesters or full-year coverage. The plan rates are very affordable. Students must be enrolled at least half-time to purchase the dental plan.
